What is a Primary Care Network?

Since the NHS was created in 1948, the population has grown, and people are living longer. Many people are living with long term conditions such as diabetes and heart disease, or suffering with mental health issues.

 

To meet these needs, surgeries are working together with local health services in groups known as Primary Care Networks, or PCNs.

To that end, the Forest of Dean PCN collaborates with local doctors and services to provide proactive and personalised health and social care for patients at the Forest GP Surgeries.
